Description of Opportunity

Interior Health and the Primary Care Network Bridge Care Virtual Clinic has a permanent, full-time opening for an experienced Occupational Therapist.

Primary Care Networks (PCNs) are community-based networks of health care professionals and clinics who plan and deliver all day-to-day health care needs of a community. Within these networks teams of providers work together closely. In this is way, patients receive better care, by the right person, at the right time, closer to home.

As the Occupational Therapist, you will be part of a newly formed Bridge Care Virtual Clinic to provide occupational therapy services to attached and unattached patients within the Williams Lake and catchment area. Office space is within in the Cariboo Community Health Centre with opportunity to see clients face-to-face, virtually, and/or occasionally within the community (for example the Alexis Creek outpost health station). The Bridge Care Virtual Clinic is run/operated by the Central Interior Rural Divisions of Family Practice.

In this position the Occupational Therapist is responsible for working in collaboration with an inter-professional care team and defined network of Primary Care Network Physicians, Nurse Practitioners, Nursing and other members of the Allied Health team (i.e. Respiratory Therapist, Dietitian, and Physiotherapist). The goal is to conduct integrated care planning, offer client assessments/clinic visits, and provide ongoing monitoring and self-management support follow-up for clients (i.e. adults, seniors, pediatrics) and/or those with multiple chronic conditions, either in an individual or group setting and often directly within the Family Physician/Nurse Practitioner. You will see clients a couple of times utilizing therapeutic inventions that contribute to promoting functional independence and enabling the client to achieve and maintain optimal health outcomes in their community setting.

The Primary Care Network Occupational Therapist has the support of the Physiotherapy Professional Practice Leader and the network of Interior Health Occupational Therapists across the Cariboo Region. Therapists come together to support one another and create linkages and connections.

This is a Day shift position on a Monday to Friday schedule 08:00 to 16:00 hours. Qualifications

• Graduation from a recognized occupational therapy program
• Current registration with the College of Occupational Therapists of British Columbia (COTBC).
• Valid BC Driver’s license where required.
