Description of Opportunity

Health Bound Health Network is a network of rehabilitation clinics and a resource centre that aims to help people overcome the challenges of disabling injury, illness, or age-related health conditions to live meaningful and healthier lives.

Objective:

Occupational therapists at Health Bound Health Network work with individuals of various ages and conditions, but predominantly our client population consists of adults and older adults with traumatic injuries such as neurological and orthopedic disorders and injuries. Our OTs are client-centered and strive to develop individualized treatment plans for their clients to help them achieve their rehabilitation goals. Health Bound OTs facilitate discharge planning for individuals who are transitioning from a hospital setting (i.e. acute care or rehab setting) to home, ensuring that their environment is accessible and safe prior to discharge and that they have all recommended services in place at the time of discharge, such as personal support services or physiotherapy services. Your day will involve a variety of tasks, such as driving to in-person sessions or assessments with clients, liaising with your PT, Psychology or RSW colleagues as part of our multidisciplinary team, sending your recommendations for assistive devices or home modifications to our administration team, and writing reports, progress notes, and or clinical notes for each of the clients in your caseloads.

Qualifications and skills needed to succeed:

  • Successful completion of B.Sc. (Occupational Therapy), or equivalent,
  • Current registration with the College of Occupational Therapists of Ontario
  • Eligibility status in C.A.O.T. (Canadian Association of Occupational Therapists)
  • Strong interpersonal, communication (both written and verbal) and organizational skills
  • Excellent problem-solving skills
  • Demonstrated ability to be self-directed, seeking direction appropriately
  • Ability to work effectively with all levels of staff and within a multidisciplinary team
  • Ability to be on-call on some evenings and weekends
  • Ability to demonstrate a customer service philosophy
  • Ability to function in a consulting model
  • Ability to travel and work on a flexible work schedule
  • ADP Authorizer preferred
  • Experience in Neurological Rehabilitation (stroke, ABI and other neurological conditions)
  • Ability to work in a manner that exemplifies the core values of Health Bound: Excellence,

  • Computer proficiency required
  • Valid driver’s licence and access to a personal vehicle

Accountabilities:

  • Assess and treat patients based on regulatory requirements
  • Complete timely and thorough clinical documentation
  • Provide education to patients, families and team members
  • Participate in multidisciplinary rounds
  • Participate in related in-service educational programs and service planning
  • Pursue continuous professional development, which includes active participation in O.T.
  • Professional Standards & Issues Committee
  • Participate in clinical evaluation
  • Participate in the development of advances in practice
