ODSP Disability Determination Package Revised
The Ministry of Children, Community and Social Services is making improvements to the
Disability Determination Package (DDP) and Medical Review Package (MRP) for Ontario
Disability Support Program (ODSP) applicants and recipients. These changes are intended to improve the ODSP
adjudication process by introducing updated forms which will be easier for health care
professionals (HCPs) and ODSP applicants and recipients to complete.
Occupational Therapists play a crucial role in providing important
medical and supporting information for people applying for ODSP or existing clients
undergoing a medical review. Their expertise and assistance are greatly appreciated, as the
ministry relies on this information to make informed decisions about whether a person is
eligible for ODSP income support.
